The Palliative Care Nurse (RN/LPN) provides compassionate, resident-centered nursing care focused on improving quality of life for residents with serious or chronic, progressive or terminal illnesses. This position works collaboratively with physicians, residents, families, and the interdisciplinary team to manage pain and symptoms, support goals of care discussions, and ensure residents receive comfort-focused care aligned with their wishes.
The Palliative Care Nurse serves as an advocate for residents and families by providing education, emotional support, and coordination of care throughout the residents healthcare journey.
The nurse is responsible for providing the daily care, health needs, safety and comfort of the residents. Including providing professional assessments and documentation of the residents health and assisting with the management of the assigned nursing units staff.
